What supporting information do DIsability Services require?
To help us put the right support and adjustments in place for your study, we ask you to provide supporting information about your disability. This information should:
- be provided by a suitably qualified professional such as a medic, specialist teacher, occupational therapist etc.
- contain clear information about the professional's organisation or job title such as a letter heading, details of the organisation in the email signature, an organisation's stamp, the individual's qualification etc.
- demonstrate that the disability is long term - lasting or likely to last 12 months or more.
- indicate the impact on your day-to-day life.
- be written in English or be a formally certified translation.
You can use the Disabled Students' Allowance evidence form and ask the professional to complete this instead of a letter or report. This can be used as supporting information whether you intend to apply for DSA or if you do not.
Please submit your supporting information to disability@hud.ac.uk along with your name and student number - thank you.
Please note, if you have a specific learning difficulty such as dyslexia and wish to apply for the Disabled Students' Allowance, your supporting information needs to be in the form of a full diagnostic report carried out by a registered psychologist, qualified specialist teacher or similar.
If you are unsure whether the supporting information you have is suitable, please submit this to Disability Services with a short explanation saying you are unsure if it is suitable. We will check this and get back in touch with you.
